athletic participation

Before your child can participate in sports team try-outs, he/she must have a Duval County Pre-Participation Athletic Screening Form completed and signed by your child's healthcare provider. This form is now available on the web by clicking here or you may get a copy from the school. There are two pages that must be completed by your healthcare provider.
